Confident Medical Insight

Placement assessments are often conducted by social workers who, while highly skilled in their field, may not have specialized health training. As a result, these assessments may lack the medical depth needed to identify certain health details. At OnitRN, our assessments are performed by registered nurses (RNs), ensuring a comprehensive, clinically informed evaluation that captures all critical health factors.

Old and Young

"The call took a little over an hour. But it didn’t feel like an hour, because the whole call was relevant to my situation and I gained a lot more knowledge. I have been going through a lot with my mom and have talked to so many new people and this was one of the best calls that gave me hope to place my mom in the correct place. My nurse was wonderful compassionate and understanding."

“Kristiana was very thorough and even did extra investigating on a topic of interest to me and my mother. She got back to me with information immediately. Thank you.”
  • How much does it cost and who pays?
The OnitRN RapidAssess is $350. Once you submit your client’s information, we’ll send a welcome letter to both you and your client, along with the invoice to your client.
  • Does Medicaid or Medicare cover the cost of an assessment?
Medicaid and Medicare do not cover the cost of placement assessments themselves. However, OnitRN's assessments are designed to provide essential and accurate information to help secure the best placement for your client, which can significantly streamline the process and potentially reduce costs associated with other services.
  • How does the process work?
Once the invoice has been paid, OnitRN will begin the process of gathering records from the client’s providers, scheduling a phone assessment with an RN and the POA or active caregivers, and assembling a comprehensive report. This report includes medical, physical, and mental health details to ensure the best placement for your client.
  • How long does the call with the RN usually take?
Every client is unique in order to cover every aspect most calls take 30 - 60 minutes.
  • How can you capture physical details without an in-person assessment?
OnitRN’s RN-led RapidAssess are conducted over the phone but are designed to gather thorough and accurate information. We rely on a combination of detailed client records, caregiver input, and direct communication with family members, along with our nurses' expert judgment, to accurately assess physical health. Our RNs ask specific questions to evaluate mobility, medical history, and current physical conditions, ensuring no key details are missed. While an in-person assessment might offer some visual cues, our process ensures that physical health is comprehensively documented using all available resources, ensuring the best placement decisions are made for the long-term well-being of the resident.
  • Which States is OnitRN RapidAssess available in?
Every state has detailed requirements and laws that must be followed. Please contact OnitRN (833) 977-6777 for RapidAssess questions in states outside of Washington.
